Privacy Act Notice

General - This notice is provided pursuant to Public/ Law 93-579, Privacy Act of 1974, 5, U.S.C. Section 552a, for individuals supplying information as data for official business for accommodating individuals as part of the Computer/Electronic Accommodation Program (CAP). The information entered into this data system becomes a part of the CAP Database and is to be kept strictly confidential. The primary use of the information is to provide assistive technology for those applying for accommodations from the CAP program. In addition to those disclosures generally permitted under 5 U.S.C. 552a (b) of the Privacy Act, these records or information contained therein may specifically be disclosed outside the DoD as a routine use pursuant to 5 U.S.C. 552a (b)(3) to the appropriate individuals who can assist with providing said accommodations.

Submission of this information is voluntary, however, failure to supply the information could result in a denial of accommodations. Unauthorized disclosure or misuse of personal information may be subject to a fine of up to $5000.
