Requesting Accommodations

Q: How do I request an accommodation?

A: CAP encourages all its customers to utilize our online Request an Accommodation process, and submit the form directly to us through our website, for processing. We also accept faxed request forms via our fax number at 703-681-9075. Supporting documentation should also be submitted via the same fax number.

Q: What signatures are required on my CAP Request Form before I submit it?

A: When submitting a paper copy of the CAP request form, your supervisors/managers or agency POC's signature is required. When submitting the request via the website, no signature is required, only the supervisors/managers or Agency POC's name, contact phone number, and primary email are needed.

Q: What do I do if my supervisor won't sign the CAP Request Form?

A: If you are unable to get the required signature, please contact CAP.

Q: What type of documentation is required?

A: Per EEOC guidance, CAP may require medical information in connection with the request for reasonable accommodations. Medical documentation includes the nature, severity, and duration of the disability and describes the extent to which the disability limits performance. Medical documentation is always requested to support repetitive stress injuries. To ensure CAP is providing the most appropriate accommodations solution, you may be required to complete additional forms such as: speech recognition form, video communication devices, personal assistant information form.
